This custodian patient datastore would be accessible/shareable from the web thru a REST API. It would manage visibility/accessibility of data thru a patient-dictated privacy rule set. It would be capable of storing not only the uniform data, but also additional data that has been semantically tagged. It would be the one place to access/post data about a patient. In this model, EHRs become the front-ends to PHI, not the gatekeepers holding data hostage. <br /><br />We are building just such a platform at Mediparency. It is our belief that storing 20% of the data will produce 80% of the value of interoperability. Passing messages around with only a partial view of the patient is inviting disaster.
